Physician report card
This sample was reviewed and approved by the ACDIS Resource Library Committee in 2023. Should you use this resource, the committee noted that this resource mainly utilizes the terms of ICD-9, and needs to be updated to reflect ICD-10 coding practices. The attached physician report card, developed by the CDS team at St. Cloud Hospital in MN, is a tool to educate physicians about Severity of Illness and Risk of Mortality scores. These scores are used for statistical purposes to rank physicians and hospitals, and they also impact risk-adjusted outcomes such as the Mortality Index, complication rate and readmission rates.
